Get Connected

For the past eleven years, Mosaic’s Get Connected Family Resource Center has operated under a grant from Carroll County. This program has assisted many Carroll County families in learning, navigating, and managing systems of care and becoming advocates for their children. When the grant funding for Get Connected services ended in 2017, the program expanded in a new direction to directly benefit our clients, Mosaic staff, and members of the community who are seeking assistance.

Get Connected functions as a comprehensive referral service, assisting Mosaic clients, staff and members of the community to connect to other services within the Mosaic system.  Additionally, should clients need assistance beyond those provided by Mosaic or the Sheppard Pratt Health System, Get Connected will help link staff and clients to resources through the program’s database system.

Are you a Mosaic client or staff member who works with clients? The Get Connected team can assist clients and staff with referrals to a broad range of services, such as:

  • Dental Care
  • Volunteering
  • Employment
  • Behavioral Difficulties
  • Developmental Concerns
  • Educational Issues
  • Mental, Behavioral Health Issues
  • Physical Health Matters
  • Substance Abuse
  • Basic needs, such as food, shelter
  • and more…..

Are you not currently part of the Mosaic system? The Get Connected team can help you! From navigating Mosaic’s many programs and services, to helping connect you with resources both across the Sheppard Pratt Health System and our communities, our team stands ready to serve as a primary source of critical information and assistance. With Get Connected, you’ll never be alone in finding the help you need.

If a Mosaic client has a need, the Get Connected team will make every effort to identify a resource to address that need. Get Connected performs a key service in Mosaic’s person-centered system of care.

The Get Connected team is focused on expanding its resource databases for all of Mosaics jurisdictions and surrounding areas. Get Connected also serves as a way to introduce Mosaic’s programs and services to people who are not currently being served by the Mosaic system. The team does this through community outreach and supports, presentations, and resource distribution, as well as connecting with local churches, police departments, government programs, schools and local hospitals across Maryland. Additionally, Get Connected works with other non-profits and local businesses to assist in addressing the needs of employees and customers.
