Our integrated team provides year-round care, including counseling services and medication therapy. We manage your or your loved one’s physical needs, address housing and employment issues, and provide other services.

Who We Serve at Chesapeake Connections

Through the right combination of mental health services and support programs, we help people achieve success in their community. 

We work with people who have one or more serious and persistent mental illnesses and have received previous psychiatric treatments:

  • Admitted to a state psychiatric hospital for six or more consecutive months
  • Admitted to an inpatient psychiatric unit at least four times in the past two years
  • Visited an emergency department for psychiatric reasons at least seven times in the past two years

Accessing Care at Chesapeake Connections

The Maryland Department of Health and Mental Hygiene supports Chesapeake Connections, which means there are no fees for the services we provide.
