Chesapeake Connections

WHAT WE DO

Chesapeake Connections provides intensive, wrap-around services 24 hours a day, year-round, for individuals who have serious and persistent mental illness. Services include, but are not limited to, mental health counseling and medication monitoring; assistance connecting with physical health care providers; supported employment; supported housing; linkages to substance abuse treatment; and recreational and social activities. The program utilizes flexible dollars to meet the individualized needs of the clients.

WHO WE SERVE

Baltimore City residents who have been in a state psychiatric hospital for 6 or more consecutive months, or have been an inpatient in any psychiatric unit at least 4 times within the past two years, or who have been seen in an emergency room at least 7 times in the past two years for psychiatric reasons.

HOW TO ACCESS AND PAY FOR SERVICES

The program is supported by the Maryland Mental Hygiene Administration. There are no client fees for direct services provided by this program.
