Family Navigator - Westminster, MD

Current Openings:

Mosaic Community Services, Inc. has an immediate opening for a Family Navigator at the Get Connected Program in Westminter, part time position. Great opportunity for a parent of a special needs child.

Duties:

Assists families who have children with special needs to navigate the system to meet various needs. Acts as public liaison between families and “the system”.

Principal Responsibilities and Duties:

1. Interviews families utilizing approved assessment tool to identify strengths, needs and then links family with appropriate services

2. Champion of local community resources in areas including physical health, mental health, development disability, education, “real life” such as hair cuts, babysitting ect. and local support groups

3. Provides case management for families of children with multiple disabilities

4. Identifies various financial resources to cover necessary services whether it be through health insurance, grants, state and federal assistance or the educational system

Requirements:

Parent of a child with special needs who has learned about the service system through personal experience.

Bachelor’s Degree preferred in related field such as psychology, community counseling or other human services field. Commensurate experience or training may be substituted for degree

Knowledge of local community resources in areas of mental health, physical health, developmental disabilities and education

Must be flexible, punctual, and available to work weekends, evenings and holidays as necessary.

Valid Driver’s license with no more than two points and provide supervisor annually with driving record.

Ability to work autonomously with demonstrated creativity, initiative, and commitment.

Knowledge of policies and procedures and the ability to follow them.

Ability to perform physical aspects of position including but not limited too lifting/carrying/moving objects up to 20 pounds, bending or reaching to assist families with their rehabilitation plans, preparing files, and working while standing/moving.
